When you create Location Extension (v201309) with only streetAddress field present (and no streetAddress2), then this extension never goes from "Eligible" status to "Approved". But when you open this extension in UI and save it without any changes, it changes into "Approved" status just fine.
I may be wrong, but looks like a problem with empty streetAddress2 field to me.
