Are you using a command line tool (such as curl) to download an adhoc 
report? 
If so, please follow these 
steps<https://adwords.google.com/api/adwords/reportdownload/v201206/reportDefinition.xsd>
.

If you are using a client 
library<https://developers.google.com/adwords/api/docs/clientlibraries>, 
it's much easier.
Only you should refer the DownloadAdHocReport example.

e.g. Java's case
http://code.google.com/p/google-api-ads-java/source/browse/examples/adwords_axis/src/main/java/adwords/axis/v201209/reporting/DownloadCriteriaReport.java
