In the AdWords UI, when you are creating an InStream video ad (template 49), you can choose an optional Companion Banner. This can be an image or a flash file.
In the AdWords API, it looks like the companion banner is only allowed to be an Image (based on this page: http://code.google.com/apis/adwords/docs/appendix/templateads.html#instream_video).
