I find it ballsy, whuhh!

I think in restores, not backups.
Consequently, I mark tapes having 
write errors for the second time 
as destroyed and throw them away,
beeing afraid of them having read errors 
in case of later restore.

I have not found a useful policy. If I start getting write errors on tapes
I make them a candidate for retirement, no matter how new or old they are.

FujiFilm LTO1 tapes in a IBM3584 library.  Does anyone have a policy (or
your thoughts) on retiring tapes, i.e. after X number of read/writes or X
years service?
