It was the semaphore settings:

This got it back up:
[root@barge ~]# sysctl -w kernel.sem="250 256000 32 16384"
kernel.sem = 250 256000 32 16384
[root@barge ~]# ps -ef | grep db2
root      3494     1  0 Apr06 ?        00:00:04
/opt/tivoli/tsm/db2/bin/db2fmcd
root     21406 30720  0 08:56 pts/0    00:00:00 grep --color=auto db2
[root@barge ~]# kill -HUP 3494
